  5. Follow the proposed course of treatment or care, recommendations and advice, which you and your provider have agreed upon.
  6. Be considerate of the rights of other patients and clients, care personnel and property.
  7. Provide accurate and timely information about sources of payment and your ability to meet financial obligations.
  8. Make it known whether you understand what is expected of you, and whether you are able and willing to comply.

  1. Explain diseases, treatments, and results in an easy-to-understand way
  2. Listen to the patients’ feelings and questions, help them make decisions about their care
  3. Keep treatments, discussions and records private
  4. Provide 24 hour access to medical care and same day appointments, whenever possible
  5. Provide instructions on how to meet your health care needs when the office is not open
  6. To care for you to the best of my abilities based on the doctors’ understanding of current medical methods available
  7. Give the patients clear directions about medicines and other treatments
  8. Send the patients to trusted experts, if needed
  9. End every visit with clear instructions about expectations, treatment goals, and future plans
